All Colorado Choice/SLVHMO members are entered into the CHOICEConnect application. All you have to do is sign in to activate your account. Begin the activation process by clicking the Members icon on the right side of the CHOICEConnect sign in screen.
2. Activate your CHOICEConnectAccount
The Healthcare
Member Registration screen
will determine if
you are a first time user or
an existing CHOICEConnect user.
Note: Parents
or Legal Guardians will have
access to dependent-child
records until the
child reaches age
18. At
that
time, the
child must grant
parental access
if the child wants
the Parents or
Guardians
to view their health
records.
Click
the Member
ID Number button
and then enter
your Colorado Choice/SLVHMO
Member ID
number in the
associated box.
Be sure to enter the two-digit suffix directly after the Member ID number. Do not leave a space or use a dash. Employee is always the "01" suffix and spouse is always "02".
Enter your birth
date in the
birth date box in mm/dd/yyyy format, for example 09/12/1955.
If you have an E-mail Address,
you will be able to send secured
messages between you and your healthcare
provider. Enter your E-Mail Address
in the associated box. If you choose
not to enter your E-Mail address
at this time, you will have the
option to add it after you gain
access to CHOICEConnect.
Click the Participating Health Plans arrow
and selectColorado Choice Health Plans(SLVHMO) from
the list.
Caution If
you fail to select a participating
health plan,
submit
an incorrect birth date, or incomplete
member
ID number, you will receive the error message "Match
Not Found." This error will prevent you from
continuing with registration.
After completing the form,
click Next. Depending on your status in CHOICEConnect,
one of three messages
will appear.

Your User ID is available so you do not need to
proceed with the registration process.
If your User
ID
is not known, contact Colorado Choice/SLVHMO Customer Service for
assistance.
If your User ID is known, follow the
Click here link to return to the sign in screen
to enter CHOICEConnect.
The Member ID and birth date entered do not match
any of the records for Colorado Choice/SLVHMO.
Follow the Click here link to return to the previous page to verify the
information entered; be sure the
correct Health Plan is selected on that page.
If
the information is correct, contact Colorado Choice/SLVHMO Customer
Service for assistance.
The information entered was either incomplete or invalid.
Click
Return to previous page to modify the information
on the previous page.
If you need further assistance,
contact Colorado Choice/SLVHMO Customer Service.
The Member ID and birth date entered match a member
in the Colorado Choice/SLVHMO records but no User ID has been issued.
Continue
with the registration process
as described in Step 3.

Establish Security Information
Complete
the fields on the registration
form. These fields
all work together to protect
your personal health information.
Click Cancel if you want to
stop the registration process.

Password
Enter a password that is at least 6 characters long,
and does not contain the User ID.
Important: Letters
used in the password MUST
be in lower case
Verify Password
Re-enter the exact same password.
This is done to
ensure you did not enter an unintended password
character and to ensure you know your
password.
Password Reminder
Enter a question that will help you remember your
password.
You cannot use your password in the question.
For example, if your password is 'kitty', your
question cannot be
'What name do I call my kitty?' However, your question
could be 'What has taken over my favorite chair?'
Security Question
Click the arrow and select one of the suggested security
questions to which only you know the answer.
This security
feature will be used by the CHOICEConnect System
Administrator to verify your identity.
Security Answer
Enter the answer to the question you selected.
When
you need to call Customer Service or the CHOICEConnect System Administrator, you will be asked your
security question and you must respond with the answer
you
provided in the Security Answer box.
Click Back or Cancel if you do not want to save the information
and leave the registration process.
Click Next to
save the information and move
onto the next screen.
Note: Failure
to complete a required field
will result
in an Error when you click
Next. If this occurs, click Return
to previous page, correct
the field specified on the
error message, and then click Next.

Registration Process Summary
After you complete the registration form and click Next, the
Registration Process Summary will appear.
Your User ID will be provided in the instructions
at the top of the Summary.
Important: Be
sure to make note of your User
ID before
you click Finish.
You have the option to
print your User Security
Agreement if your computer
is connected to a printer.
Click Print to print the Security agreement. Click Finish.
You will be returned to the
Sign In screen at which time you
can sign in to CHOICEConnect.
You can
print
the User Security Agreement from this screen if your
computer is connected to a printer.
5.
Obtain Adobe Acrobat Reader
CHOICEConnect provides some reports to help
you manage
your healthcare
benefits. To view these
reports, Adobe Acrobat
Reader must be installed on your
computer.
The
basic version of the
program
is available to all internet
users
at no cost.
After you
click the Members icon on
the Sign in screen,
the next
screen in the registration
process will open.
If you need to install
Adobe Acrobat Reader,
click the icon below on the screen
and
then follow
the Adobe Acrobat Reader
instructions to install
the program.
6. System Requirements
CHOICEConnect,
a web-based application which

through any internet connection
