COLORADO CHOICE MEDICARE PLANS (COST)
Colorado Choice’s Medicare Cost Plans are offered in 16 of the most rural and underserved counties in Colorado including Alamosa, Bent, Chaffee, Conejos, Costilla, Crowley, Custer, Fremont, Huerfano, Las Animas, Mineral, Otero, Prowers, Rio Grande, Saguache, and Yuma Counties.
Colorado Choice provides all Medicare-Covered Benefits. To qualify, you must reside in one of the above counties, be eligible for Part A, be enrolled in Part B and not have End Stage Renal Disease. You will still need to continue payment of your Part B premium. Our plans do NOT cover Part D or drug benefits.
You may enroll in our plan:
When you first become eligible for Medicare (three months prior to the month you turn 65 to three months after the month you turn 65)
If you become eligible for Medicare due to a disability (three months before and after you receive 24 months of cash disability benefits)
During annual Medicare open enrollment periods
At other times, due to special circumstances
And individuals enrolled in Original Medicare may chose to enroll in one of Colorado Choice’s Medicare Plans any time throughout the year

You are eligible to enroll if you are entitled to Medicare Part A and enrolled in Medicare Part B and you live in the service area. You must continue to pay your Medicare Part B premium if not otherwise paid for under Medicaid or by another third party. Eligible beneficiaries can enroll in Colorado Choice at any time.
You can use any doctor who is part of Colorado Choice's network. You may also go to doctors outside of our network. We may not pay for services you receive outside of our network, but Medicare will pay for its share of charges it approves. You will be responsible for paying the Medicare deductible and coinsurance for those services, unless they were authorized in advance by Colorado Choice.
