If you are a Colorado Choice Health Plan or San Luis Valley HMO member, then you are already registered to use ePHIT! All you need to do is click the "Login" link in the navigation on the left or below.
How to Enroll:
1. To enroll go tohttps://my.ephit.com/mywellchoice/ 2. Click the "Member log in" button located on the blue toolbar at the top
3. Click the orange "Register Today" button
4. Enter your first/last name and last 4 digits of your member ID #
5. Complete the registration and setup questions
6. Set up your GetFIT, EatFIT, and LiveFIT plans and start seeing results today!
