If you are a Colorado Choice Health Plan or San Luis Valley HMO member, then you are already registered to use ePHIT! All you need to do is click the "Login" link in the navigation on the left.
How do I Login?
Once you get to the Login page, enter you First Name, Last Name and your Member ID# (9 digits) and suffix (2 digits). Do not use hyphens or spaces in number. [Ex. 99999999901]). To locate your Member ID number, please refer to your health plan ID card.
How do my children Login? Children 18 or older, login using the Member ID# and the childs suffix which will be 03 or higher. Children under the age of 18 will login with a parent, under the parents Member ID# with suffix 01 or 02, depending on which parent is logging them in.
